Morbidity and Mortality Conference - Anesthesiology

1. Analyze contributing factors to medical errors and near misses, including system failures, communication breakdowns, and safety culture, and explain principles of patient safety and error prevention in perioperative and procedural care.
2. Apply structured communication and error analysis strategies during case review and demonstrate effective, non- punitive discussion techniques to promote transparency, teamwork, and shared learning within the healthcare team.
3. Implement system-based improvements identified through M&M review, evaluate contributing factors to adverse events, and improve team-based practices to reduce preventable harm and enhance patient safety outcomes.

In support of improving patient care, this activity has been planned and implemented by Texas Tech University Health Sciences Center and ______. Texas Tech University Health Sciences Center is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC), to provide continuing education for the healthcare team.
